The Cook County FMRP is committed to providing quality health care to Cook County's medically underserved and diverse communities. Our patient population is heavily impacted by the Social and Economic Drivers of Health . Our program offers special expertise in managing patients with complicated medical and biopsychosocial problems in both tertiary care inpatient and PCMH ambulatory settings.
